Job summary

Age UK Lancashire is seeking a Service Coordinator for the Living Well Support Service, covering the East Lancashire area. You will promote independent living, coordinate referrals and manage a small team of Wellbeing Workers to deliver timely support after hospital stays.

The role is permanent, 21 hours per week with some weekends and bank holidays. It is office-based but includes time in hospitals and the community, with comprehensive training and a friendly, supportive culture.

Qualifications

• Excellent communication skills, both verbal and written.
• Demonstrated decision-making and problem-solving abilities.
• Ability to maintain professional boundaries.
• Strong organisational skills with workload prioritisation.
• Digital skills to manage time efficiently.
• Full valid driving licence and use of a car for business.

Responsibilities

• Promote independent living and coordinate referrals within the Living Well Service.
• Manage a team of Wellbeing Workers, scheduling and overseeing their work and targets.
• Use a solutions-focused approach to help clients access support from Age UK Lancashire or others.
• Oversee assessments on hospital wards and support discharge planning.
