Job summary

Age UK Lancashire is recruiting a Service Coordinator for the Living Well Support Service across the Central Lancashire area to oversee falls prevention and frailty pathways. The role involves coordinating referrals, assessments, and partnerships with health, social care and community stakeholders.

The post includes supervising a small team of Wellbeing Workers and ensuring safe, timely in-home support while maintaining professional boundaries and strong organisational skills.

Responsibilities

• Coordinate the delivery of a targeted falls prevention and frailty pathway for older people in a specific area.
• Manage referrals, oversee assessment activity, and build relationships with health, social care and community partners.
• Supervise and support a small team of Wellbeing Workers to ensure service delivery targets are met.
• Apply a solutions-focused approach so people can access help from Age UK Lancashire or other community-based support.
