Job summary

Age UK Lancashire is seeking a Service Coordinator to join the Living Well Support Service across the East Lancashire area. The role involves promoting independent living, coordinating referrals, and managing a small team of Wellbeing Workers to support hospital discharges and home-based assistive care.

You'll deliver a responsive, community-focused service, ensuring timely access to help, building relationships, and working across hospitals and community settings.

Qualifications

• Excellent communication skills, both verbal and written.
• Ability to demonstrate effective decision-making and problem-solving.
• Ability to maintain professional boundaries.
• Strong organisational skills with the ability to prioritise workloads to meet deadlines.
• Hold a full valid driving licence and use of a car for business purposes.

Responsibilities

• Promote independent living and coordinate referrals within the Living Well Service.
• Manage a team of Wellbeing Workers, scheduling and overseeing their work, providing support as required, and ensure service delivery targets are met.
• Use a solutions-focused approach to ensure people can access help when needed from Age UK Lancashire or community-based services.
