Job summary

Age UK Lancashire’s Living Well Support Service seeks a Service Coordinator to work across Preston, Chorley and South Ribble. You will promote independent living, coordinate referrals and lead a small team of Wellbeing Workers to deliver timely post-hospital support.

This role focuses on discharge planning, signposting to community services and building strong relationships with hospitals and clients. A solutions‑focused approach, good communication and organisational skills are essential;

Qualifications

• Experience coordinating referrals within a healthcare/community support service.
• Experience leading a small team of wellbeing workers.
• Excellent communication and relationship-building skills.
• Ability to manage workloads and meet deadlines.
• Full driving licence and access to a car for work.

Responsibilities

• Promote independent living and coordinate referrals within the Living Well Service.
• Manage a team of Wellbeing Workers, scheduling and overseeing their work to meet targets.
• Use a solutions-focused approach to help clients access support from Age UK Lancashire or partners.
• Supervise assessments on hospital wards, support discharge, arrange home from hospital and signpost services.
• Maintain professional boundaries and build relationships with hospitals, clients and community partners.
