Job summary

Age UK Lancashire’s Living Well Support Service is hiring a Service Coordinator to work across Preston, Chorley & South Ribble. The role focuses on promoting independence, coordinating referrals from diverse sources, and leading a small Wellbeing Worker team for timely service delivery.

You will support hospital discharge, arrange in-home follow-up, and signpost to other services when needed. The post is based at Royal Preston Hospital with 28 hrs/week.

Qualifications

• Must have strong communication skills, both verbal and written.
• Full valid driving licence and use of a car for business purposes.
• Role emphasizes independence, problem solving and ability to work to deadlines.

Responsibilities

• Promote independent living and coordinate referrals within the Living Well Service.
• Manage a small team of Wellbeing Workers and oversee scheduling to meet targets.
• Assist hospital discharge and arrange short-term in-home support as required.
