Job summary

Lancashire County Council's Adult Social Care service is recruiting Social Workers to support hospital discharge processes in Preston. You will work with health colleagues, families and partners to plan safe, timely and person‑centred discharges from hospital to the most suitable setting, promoting independence and recovery.

Qualifications

• Registration with Social Work England (SWE) is required.
