Age UK Lancashire is recruiting two Wellbeing Workers for the Living Well Support Service to assist hospital discharges and help clients regain independence across Preston, Chorley and South Ribble. Roles based at Preston Royal Hospital and Chorley General Hospital, with community visits typical.
You will take patients home from hospital, provide up to 6 weeks of follow-up, and use a solution-focused approach to ensure timely access to support. A full driving licence is essential.

Age UK Lancashire’s Living Well Support Service provides invaluable support to clients over the age of 18, helping them attain or regain independence after a hospital stay or prolonged illness. Our service is integral in supporting hospital discharges, preventing unnecessary hospital admissions, and may reduce the need for residential care. In the last 12 months, the service has supported over 9,000 people across the County.

One role is based at Preston Royal Hospital and the other at Chorley General Hospital, you will make community visits across the Preston, Chorley and South Ribble areas, working in hospitals and client’s homes. You will be required to attend meetings and spend time at our offices working with Age UK Lancashire colleagues.
