Job summary

Age UK Lancashire is seeking a Service Coordinator for the Living Well Support Service in the Preston area. The role focuses on coordinating referrals, discharging patients from hospital, and guiding adults to regain independence after illness.

You will oversee a small Wellbeing Worker team, ensuring timely, compassionate support. The position is predominantly office-based with hospital/community duties and requires a valid driving licence.

Qualifications

• Excellent communication skills, both verbal and written.
• Full valid driving licence and use of a car for business purposes.
• Experience leading a small team and coordinating care/workforce.

Responsibilities

• Promote independent living and coordinate referrals within the Living Well Service.
• Manage a team of Wellbeing Workers and oversee service delivery targets.
• Arrange discharge support from hospital and provide short-term in-home support when required.
