Qualifications

• Demonstrable literacy and numeracy to Functional Skills Level 1 or equivalent.
• NVQ Level 2 in a care-related subject or equivalent, or willingness to undertake Level 2 Apprenticeship
• GCSE Grade C or above or Functional Skills Level 2 or equivalent in English and Maths

Responsibilities

• Support the Specialist Forensic and Community Wellbeing Team, primarily within the Severe Mental Illness pathway and, where required, the Medication Management pathway.
• Deliver high-quality healthcare support under the direction of the Nursing Team and Allied Health Professionals within a primary care setting.
• The role will focus mainly on the SMI pathway, completing physical observations and physical health checks, while also supporting adults experiencing mental health difficulties.
• The post holder will maintain accurate paper and electronic records, including waiting lists and SMI registers, and report risks or changes in patient presentation promptly.
