Job summary

Nottinghamshire Healthcare NHS Foundation Trust is recruiting for a Community Support Worker. The role involves working with adults in the community, conducting assessments and delivering evidence-based interventions as part of City Local Mental Health Teams.

You will join a multidisciplinary team, promoting recovery and coordinating with primary care and partner agencies to support safe discharge. Full UK driving licence is preferred and DBS checks apply.

Qualifications

• Evidence of a good basic education and basic IT skills.
• Excellent communication and interpersonal skills; ability to work in a multidisciplinary team.

Responsibilities

• Contribute to assessments and interventions in the home/community for adults with mental health needs.
• Promote recovery and coordinate with partner agencies and primary care to support discharge from secondary care.
• Work with a dynamic multidisciplinary health team and manage complex cases with other agencies.
