Job summary

Cheshire East Council is recruiting a Senior Support Worker for the Care4CE Mental Health Reablement team in the Macclesfield area. The role is full-time (37 hours) and permanent, with some early-evening work.

The service delivers up to 6 weeks of practical support to adults facing mental health issues and related challenges. Based at Macclesfield Town Hall and covering Congleton, Wilmslow and Macclesfield, you will assess risk, support home visits, and coordinate referrals into community

Qualifications

Experience in health or social care setting with understanding of mental health.
Ability to assess risk and follow safeguarding procedures.
Knowledge of local service provision.
Excellent communication, organisation and record-keeping skills.
Enthusiastic, flexible, reliable.
Able to support wider team and be a point of contact for stakeholders.

Responsibilities

Make appointments at client home addresses and support referrals to engage in the community.
Provide short-term practical support to adults with mental health, debt, housing, education or social inclusion issues.
Travel to home visits with mileage paid for car use.
Cover the Congleton, Wilmslow and Macclesfield area in north Cheshire East.
