Job summary

Cheshire East Council is recruiting for a permanent Senior Support Worker in the Care4CE Mental Health Reablement team, based in Macclesfield. The role is 37 hours per week, Monday to Friday with occasional early evenings, covering Congleton, Wilmslow and Macclesfield.

You will deliver short-term practical support to adults with mental health and related issues, with travel for home visits paid via mileage.

Qualifications

• Experience in health or social care and understanding of mental health issues.
• Ability to assess risk and apply safeguarding procedures.
• Knowledge of local service provision and available support pathways.
• Excellent communication, organisation and record-keeping skills.
• Enthusiastic, flexible and reliable with a team-focused approach.
• Ability to act as a point of contact for stakeholders and support the wider team.

Responsibilities

• Make appointments at the home address for clients.
• Support referral to engage in community activities and services.
• Keep accurate records and document care plans and contacts.
• Assist with addressing safeguarding concerns and risk assessments.
• Utilise mileage allowance when traveling to home visits.
• Support the Mental Health Reablement team in day-to-day tasks.
