Qualifications

• Previous experience within a health and social care setting.
• Confidence in implementing support plans, risk assessments, and safeguarding procedures.
• Strong communication skills and the ability to build positive relationships with families and professionals.
• The ability to support and mentor colleagues within the team.

Responsibilities

• Deliver high-quality, person-centred support to individuals with complex needs.
• Lead by example, supporting day-to-day running and ensuring safe, compassionate care.
• Mentor and guide Support Workers to maintain high standards of care and positive outcomes.
• Build relationships with families, healthcare professionals and external agencies to promote independence.
