Job summary

HCRG Care Group is seeking a Band 7 Speech and Language Therapist to join the Early Supported Discharge Stroke Team at Gravesham Community Hospital, Gravesend. You will deliver specialist assessment, diagnosis and rehabilitation for adults with communication and swallowing disorders as they transition from hospital to home.

You will work autonomously within a multidisciplinary team, develop personalised care plans, support discharge planning and contribute to service development.

Qualifications

• Degree in Speech and Language Therapy required.
• Registration with HCPC mandatory.
• Membership of the Royal College of Speech and Language Therapists (RCSLT) preferred.
• Dysphagia training and competence to independently manage swallowing disorders.
• Significant post‑registration experience with adults after stroke.
• Experience in community, rehabilitation or stroke services.
• Experience supervising junior staff, students or rehab assistants.
• Excellent knowledge of assessments and interventions for acquired communication and swallowing disorders.

Responsibilities

• Deliver specialist assessment, diagnosis and rehabilitation for patients with communication and swallowing difficulties after stroke.
• Manage a complex caseload within the Early Supported Discharge service.
• Develop patient-centred treatment plans and rehabilitation goals.
• Support discharge planning and work with families, carers and partner organisations.
• Provide leadership, mentoring and training to colleagues and rehabilitation staff.
• Contribute to service development, audits and quality improvement initiatives.
