Job summary

Derbyshire Community Health Services NHS Foundation Trust is recruiting an Experienced Specialist Speech and Language Therapist to join an Early Supported Stroke Discharge (ESSD) initiative and the Adult Community SLT team. The role covers High Peak, North Dales, Chesterfield, and North East Derbyshire with a 0.4–0.6 WTE split across sites.

The post requires HCPC registration and RCSLT membership, with leadership experience and a passion for delivering high-quality, evidence-based stroke

Qualifications

• HCPC-registered SLT and RCSLT membership.
• Demonstrated leadership in service development and clinical pathways.

Responsibilities

• Deliver ESSD and adult community SLT care across Derbyshire sites.
• Lead and influence team members to ensure high-quality, evidence-based stroke rehabilitation.
• Provide patient-centred care and contribute to development of new ESSD model.
