Job summary

HCRG Care Group is seeking a Band 6 Physiotherapist to join the Early Supported Discharge team at Gravesham Community Hospital, Gravesend. You will deliver rehab in patients' own homes, manage a busy caseload, and work within a supportive MDT to promote safe discharge and recovery.

The role emphasizes stroke and neurology rehabilitation, with opportunities for supervision, learning and career progression within a national provider known for quality care.

Qualifications

• HCPC registration and CSP membership required.
• Post-registration experience in neurology, rehabilitation or community care.
• Ability to manage a clinical caseload with evidence-based interventions.
• Strong communication and multidisciplinary teamwork.

Responsibilities

• Assess, plan and deliver physiotherapy for neuro conditions in the ESD service.
• Manage a caseload of patients (including stroke) and support discharge.
• Collaborate with multi-disciplinary colleagues and external partners.
• Supervise junior staff and therapy assistants when needed.
• Document clinical activities accurately and timely.
• Support service development and quality improvements.
