Job summary

Service Care is seeking a Senior Social Worker to join the Locality Team West on a two-week contract in Lancashire. The role focuses on adult assessments and care management within a supportive, professional environment.

Immediate start is available, with opportunities to gain local experience. The ideal candidate will have HCPC registration, proven experience in adult assessment and care planning, and the ability to work independently or within a multidisciplinary team.

Qualifications

• Proven experience in adult assessment and care management.
• Strong understanding of social work principles and legislation.
• Registration with the HCPC (Health and Care Professions Council).

Responsibilities

• Conducting thorough adult assessments to determine individual needs.
• Developing and implementing care plans to support client independence.
• Collaborating with multidisciplinary teams to coordinate care services.
• Providing expert advice and guidance to junior team members.
• Ensuring compliance with safeguarding policies and legal frameworks.
• Maintaining accurate and detailed case records.
• Participating in team meetings and contributing to service improvement initiatives.
