Job summary

Leaders in Care is recruiting an Adult Locality Social Worker to join a busy, community-based service. The role offers hybrid working with just 1–2 days in the office, plus visits to the community to support adults locally.

You will complete Care Act assessments, manage complex caseloads, undertake safeguarding enquiries and work with families, health professionals and partner agencies to promote independence and good outcomes.

Qualifications

• Active Social Work England registration.
• Experience in Adult Social Care, preferably within a locality or community team.
• Strong assessment, safeguarding and care-planning skills.
• Confidence managing a busy caseload and undertaking community visits.
• A recognised Social Work qualification.

Responsibilities

• Completing Care Act assessments, reviews and support plans.
• Managing complex caseloads within the community.
• Undertaking safeguarding enquiries and risk assessments.
• Working closely with families, health professionals, providers and partner agencies.
• Supporting adults to access the right care, services and resources.
