Service Care is seeking a Social Worker for the West Locality on a 2-week contract to support older adults. You will manage care assessments, safeguarding enquiries, and duty tasks while coordinating with family and multidisciplinary teams to deliver tailored care.

Qualifications

• Ideally, at least 3 years’ experience working with older people in a care management capacity.
• Experience completing safeguarding enquiries and Care Act assessments
• Ability to handle duty tasks with professionalism and efficiency
• A clear DBS - enhanced check
• Good organisational and communication skills
• Flexibility to work within a 2-week contract timeframe

Responsabilités
