About the role

As a Senior Social Worker, you will play a key role in improving mental health outcomes for Hillingdon residents. You will work in close collaboration with residents, families, carers, and multidisciplinary teams to provide person-centred, recovery-focused support.

Key Responsibilities:

• Complete Care Act Assessments
• Undertake Mental Capacity Act assessments
• Make placement requests and manage the Reablement / Discharge to Assess (D2A) process
• Handle complex cases, including housing referrals and homelessness applications
• Work in partnership with health professionals such as Hospitals and CCGs
• Attend and contribute to multi-disciplinary team meetings, family meetings, and best interest meetings
• Carry out Section 42 safeguarding investigations
• Provide supervision to both newly qualified and unqualified practitioners within the team
• Undertake Practice Educator training, where relevant
