Leeds Community Healthcare NHS Trust is recruiting a Senior Nurse to join the Neighbourhood Response Team in Leeds. The role involves providing rapid assessment and treatment in patients’ homes, leading clinical care and working with multidisciplinary colleagues to deliver safe, effective care.

You will operate within 0-2, 2-4, and 4-48 hour response timeframes, supporting early discharge and preventing unnecessary hospital admissions.

Qualifications

• Senior Nurse with clinical leadership and care coordination responsibilities.
• Experience in community/urgent care settings.
• Ability to work across MDTs and with partners.

Responsibilities

• Provide rapid assessment and treatment in patients' homes within 0-2/2-4/4-48 hour timeframes.
• Provide clinical leadership and support service development.
• Collaborate with multidisciplinary colleagues to deliver safe, effective, and compassionate care.
• Support governance, safety and patient experience improvements.
