Job summary

Central and North West London NHS Foundation Trust seeks an experienced Advanced Nurse Practitioner to lead rapid response in the community. You will conduct comprehensive assessments, make autonomous clinical decisions and supervise a multidisciplinary team to safely avoid unnecessary hospital admissions.

You will deputise for the pathway lead, coordinate urgent referrals, and work with GPs and social care to deliver high-quality, person-centred care within a 14-day episode framework.

Qualifications

• Must be an advanced nurse with eligible NMC registration and ready to provide senior clinical leadership.
• Provides mentorship to Band 6 and junior colleagues and leads on complex assessments.
• Makes advanced autonomous clinical decisions and oversees MDT reviews.

Responsibilities

• Lead on clinical assessment and management of patients with complex/acute needs.
• Provide leadership, supervision and mentorship to team members.
• Coordinate rapid response and hospital avoidance pathways in the community.
