Job summary

Oxleas NHS Foundation Trust is seeking an Adult Physical Health Senior Staff Nurse to join the Rapid Response Team in the community and hospital settings. The post focuses on rapid 2-hour assessments, integration with social care, and working across multiple sites to prevent unnecessary hospital visits.

37.5 hours weekly with shift coverage. The role requires extensive community nursing experience, NMP and prescribing qualifications, and the ability to work in a multidisciplinary team.

Qualifications

• Three years post registration experience / working in the community
• NMP Qualification V300 required
• Community Practitioner Prescribing V160 required

Responsibilities

• Conduct comprehensive nursing assessments for patients in acute and community settings.
• Undertake delegated line management of junior staff.
• Work as part of the Rapid Response Team to provide same day urgent care in the community.
• Identify appropriate onward referrals and initiate integrated care pathways.
• Maintain up-to-date clinical records and RiO system entries.
