An exciting opportunity has arisen for an enthusiastic and motivated Senior Nurse to join our developing Neighbourhood Response Team at Leeds Community Healthcare. This is a fantastic time to become part of a service that is growing, evolving and helping to shape the future of urgent community care across Leeds.

The Neighbourhood Response Team is a key part of delivering care closer to home, supporting people to remain independent, avoid unnecessary hospital admissions and return home safely following discharge from hospital.

Working across response timeframes of 0-2 hours, 2-4 hours and 4-48 hours, we provide timely, person-centred nursing care in patients' own homes, ensuring they receive the right care, at the right time, in the right place.

As a Senior Nurse, you will provide clinical leadership, support service development, and work alongside multidisciplinary colleagues to deliver safe, effective and compassionate care. This varied role offers the opportunity to utilise a wide range of clinical skills while contributing to the ongoing development of an innovative and expanding service.

We have bases across Leeds and offer flexible shift patterns between 7am and 6pm, seven days a week.

In return, we offer a supportive team environment, personalised development opportunities and a clear career progression pathway.

As a Senior Nurse within the Neighbourhood Response Team, you will provide rapid assessment, intervention and treatment to patients within 0-2 hour, 2-4 hour and 4-48 hour response timeframes, helping to prevent unnecessary hospital admissions and support timely discharge from hospital.

You will work closely with Community Matrons, District Nurses, Staff Nurses and Neighbourhood Clinical Assistants, alongside a wide range of services including Neighbourhood Nursing, Neighbourhood Therapy, Neighbourhood Out of Hours and Active Recovery to deliver seamless, coordinated care.

The role requires strong clinical decision-making skills and the ability to work collaboratively with internal and external partners to provide safe, effective and person-centred care within patients' homes. You will support individuals to achieve their goals, maximise independence and promote self‑management wherever possible.

As part of a fast‑paced and evolving service, you will demonstrate resilience, adaptability and a commitment to delivering high‑quality care. You will contribute to service development, support innovation and continuous improvement, and champion excellent patient experience, safety and clinical governance.

You will also act as a role model for colleagues, providing leadership, support and professional expertise while helping to shape the future of community‑based urgent care across Leeds.

Please review the Job Description and Person Specification attached for full requirements of the role.
