Zusammenfassung

Bristol City Council is seeking two Senior Reablement Support Assistants for the South or East Reablement Service in Bristol. You will supervise reablement staff and oversee care elements to help service users regain independence after illness or injury.

The role focuses on enabling early hospital discharge, home-based assessments, goal setting, and progress reviews, with a flexible, team-driven approach.

Qualifikationen

• Experience providing care and support to vulnerable adults within the community.
• NVQ/QCF Level 3 qualification (or willingness to undertake this while in post).
• Strong communication and interpersonal skills.
• Experience supervising or motivating colleagues.
• Ability to carry out assessments, set goals and review progress with service users at home.
• Flexible and proactive approach to changing priorities and individual needs.
• Confidence working independently as well as in a team.
• Good IT skills and ability to use digital systems.

Aufgaben

• Supervise reablement support assistants and manage the care element of service users' time.
• Support people to leave hospital earlier and maximise independence.
• Work with service users in their own homes for up to 42 days to promote confidence.
