Honiton Surgery is seeking an Experienced Practice Nurse for a full-time role, with flexibility for 30+ hours or compressed hours for the right candidate. A supportive management team and lead nurse back you up in delivering high-quality patient care.

The post involves chronic disease management, IPC compliance, immunisations, and participation in audits and governance within a multidisciplinary team.

Qualifications

• Experience of working as a practice nurse.
• Experience of chronic disease management.
• Experience of working autonomously.
• Registered Nurse with UK registration.
• Postgraduate Diploma or Degree in Chronic Disease Management.
• Desirable: Mentor or teaching qualification.
• Desirable: Nurse prescriber.

Responsibilities

• Support the Infection Prevention Control (IPC) lead and follow IPC policy.
• Consult with patients and families to assess needs.
• Manage long-term conditions and contribute to quality improvement.
• Develop and embed care management for frail and housebound patients.
• Support public health and screening programmes including immunisations and cervical screening.
• Lead on public health campaigns (e.g., flu clinics) and wound care.
• Coordinate the nursing team rota and staffing to meet patient needs.
• Review and implement nursing protocols with partners.
• Attend annual appraisals and participate in training objectives.
