Job summary

Oakley Medical Practice in Leeds seeks a salaried lead practice nurse to join a multidisciplinary primary care team within the South and East Leeds GP Group. The role focuses on long-term condition management, immunisations, wound care, and non-medical prescribing where appropriate.

You will work across sites, support quality improvement initiatives, and supervise nursing staff while maintaining high standards of patient care and record keeping.

Qualifications

• First level degree in nursing
• Diploma in chronic disease management (respiratory, diabetes, CHD) or willingness to pursue one
• Qualifications or signed competencies in phlebotomy, immunisation, wound management, health screening and travel health, or willingness to work towards them

Responsibilities

• Manage long-term conditions (asthma, COPD, diabetes, CHD, hypertension) and NHS health checks
• Deliver immunisations and manage vaccine stock
• Provide wound/ulcer care and Doppler assessment
• Assist with contraception, device fitting, cervical cytology, minor surgery support
• Carry out phlebotomy, venepuncture, ECGs, urinalysis, new patient medicals
• Request pathology tests and administer medicines under prescribing frameworks
• Provide travel health advice and chaperone during examinations
• Maintain accurate records to NMC standards and share information with the team
• Follow protocols and participate in quality improvement and meetings
