Qualifications

• NMC registered nurse with experience in primary care.
• Experience delivering chronic disease reviews and wound care.
• Familiarity with EMIS, SystmOne or Vision preferred.

Responsibilities

• Conduct chronic disease reviews for asthma, COPD, diabetes or hypertension.
• Perform wound care, dressings and treatment room duties.
• Carry out immunisations (if trained) and cytology screening as applicable.
• Undertake NHS Health Checks and lifestyle assessments.
• Manage results, recalls and routine follow-up tasks.
• Document clearly in the clinical system.
