Oakley Medical Practice in Leeds is seeking a practice nurse to join the primary healthcare team. You will provide care for patients, including long-term condition management, health screening, immunisations, treatments and dressings as part of everyday clinical activity.

You will hold an NMC-registered general nurse qualification and a Diploma in chronic disease management, with readiness to pursue additional competencies as required.

Qualifikationen

• Registered General Nurse with current NMC registration.
• Diploma in chronic disease management with relevant conditions.
• Signed competencies in the activities listed and willingness to work towards any not yet obtained.

Aufgaben

• Provide care for long-term condition management.
• Conduct health screening and health promotion.
• Administer immunisations, treatments and dressings; wound management.
• Phlebotomy where required and relevant clinic duties.
• Liaise with multidisciplinary team to meet patient needs and promote wellbeing.
