Job summary

Qualifications

• Educated to degree level or equivalent within the NHS.
• Recognised Patient Safety Incident Response Training.
• Evidence of continuing professional development.

Responsibilities

• Lead and support the patient safety function across the Trust.
• Promote robust incident reporting and harm grading per LfPSE definitions.
• Lead PSIRF investigations using SEIPS with clinicians and executives.
• Drive SMART action plans to mitigate recurrence and escalate blockers.
• Provide clinical and analytical expertise to patient safety intelligence.
• Produce assurance reports for Board Committees with data interpretation.
