Job summary

Alder Hey Children’s NHS Foundation Trust in Liverpool is seeking a Patient Safety Incident Investigation Lead (NHS AfC Band 8a) to provide expert oversight of complex patient safety investigations across the Trust, and to assure the quality and effectiveness of the PSIRP and quality framework.

The role emphasizes learning, human factors, compassionate engagement with children and families, and measurable improvements in safety and care.

Qualifications

• Masters level education in a relevant field or equivalent experience.
• Specialist training in patient safety investigation or equivalent.
• Trained in Human Factors principles.
• Evidence of continuing professional development.
• Registration with a relevant regulatory body where applicable.

Responsibilities

• Lead highly complex, sensitive investigations using PSIRF methodologies.
• Embed human factors principles and promote organisational learning.
• Translate investigation findings into measurable quality improvements.
• Engage patients, families and staff to ensure voices are heard.
• Collaborate with subject matter experts to maximise impact.
