Job summary

Derbyshire Community Health Services NHS Foundation Trust is seeking an Advanced Therapy Support Worker to join our Rapid Response Team. The role involves rehabilitation support in the community and across care settings, working alongside qualified therapy staff to prevent admissions and support discharges.

Based in Staveley Centre, Inkersall Chesterfield, you will visit patients in their homes and care homes and travel between settings as required.

Qualifications

• Experience in rehabilitation, physiotherapy or occupational therapy support work.

Responsibilities

• Conduct initial assessments and reviews of patients and create patient-led care plans.
• Meet therapy goals in line with competencies and liaise with other specialist services.
