Avon & Wiltshire Mental Health Partnership NHS Trust in Bristol is seeking a Band 6 Specialist Recovery Practitioner to provide evidence-based interventions in the community, supporting service users and their supporters.

The post includes clinical leadership within a multidisciplinary team, personalised care planning, risk assessments, and delivery of recovery-focused interventions. A registered mental health nurse with NMC is required.

Formación

• Registered mental health nurse with NMC registration.
• Experience working in community mental health settings.
• Experience working within a recovery team.
• Experience of medication administration.
• Experience delivering psychological interventions.

Responsabilidades

• Actively initiate collaborative assessments, goal setting and planning with service users and families.
• Identify service user needs and plan individualised nursing care using contemporary assessment tools.
• Influence developments to improve the service user experience and advocate on their behalf.
• Provide biopsychosocial interventions and assess safety needs with risk planning.
• Record risk incidents and co-create Safety Assessments and Safety Plans.
