Job summary

Avon & Wiltshire Mental Health Partnership NHS Trust in Chippenham is seeking a Band 6 Specialist Recovery Practitioner for a community role with a base in Chippenham and Devizes. The post requires a registered mental health nurse with clinical experience in severe mental illness.

You will work within NEW CMHT as part of a multidisciplinary team to deliver personalised care and lead on risk management and recovery planning.

Qualifications

• Must have a UK driving licence.
• Demonstrates significant experience of, and relevant professional practice.
• Experience of working within mental health services.
• Demonstrates significant experience of assessing risk and developing risk management strategies.

Responsibilities

• Deliver an ongoing comprehensive mental health service.
• Facilitate Personalised Care and Support Planning.
• Record risk incidents and collaboratively create/update Safety Assessments and Plans.
• Provide personalised interventions to support people with Mental Health difficulties.
• Act as a Keyworker for people receiving interventions.
• Plan, deliver and evaluate defined, non-profession specific specialist therapeutic interventions.
• Plan, deliver and evaluate Nursing Interventions as part of a Multi-Disciplinary Team.
• Work within a MDT, providing leadership and collaborating with other professionals.
• Deliver activities/interventions to support Carers.
• Manage own workload and plan others' workload; utilise diaries/scheduling.
• Protect individuals from abuse and harm per safeguarding policies.
• Coordinate protective interventions as required by protection plans.
• Provide active clinical leadership and supervision within the team.
• Maintain the health and social care record (paper and electronic).
• Report timely activity related to information and performance requirements.
• Participate in management, caseload and clinical supervision.
• Provide mentoring/training to others and support student learning objectives.
