Job summary

Midlands Partnership NHS Foundation Trust in Shropshire is seeking a Band 6 Mental Health Practitioner to join our Community Mental Health Services in North Shropshire Locality.

You will work within a dynamic multi-disciplinary team to deliver specialist assessments, care planning and interventions for adults with functional mental illness, collaborating with service users, carers and partner agencies to promote recovery and independence.

Responsibilities

• Provide specialist assessment and care planning in conjunction with wider community services, service users and carers
• Evaluate treatment plans
• Triage and assess referrals received by the service in liaison with multi-disciplinary colleagues
• Co-ordinate the work of the Team
• Take responsibility for managing a caseload, including complex caseloads and care planning
• Participate in setting quality standards and auditing of practice in line with clinical guidance and policy
• Administer medication (Nurses only), complying with Trust procedures and NMC Guidance
• Assess patients’ individual care needs, develop, implement and evaluate programmes of care
• Deliver a range of interventions including pharmacological and psychologically informed therapies
• Complete robust risk assessments and risk management plans, promoting positive risk taking to improve quality of life
