NHS Jobs seeks a Band 6 Specialist Recovery Practitioner in Chippenham to deliver a comprehensive mental health service within a multidisciplinary team. You will facilitate personalised care planning, record risk and safety plans, and provide targeted interventions for individuals with mental health needs.

You will also lead on clinical supervision, act as a mentor for students, and coordinate workload within the team, ensuring high-quality, person-centred care in line with trust policies.

• 1. Deliver the full range of activities to provide an ongoing comprehensive mental health service.
• 2. Facilitate Personalised Care and Support Planning in line with Your Team Your Conversation and Your Plan guidance.
• 3. Record risk incidents and collaboratively create/update Safety Assessments and Safety Plans.
• 4. Provide personalised interventions to support people with Mental Health difficulties.
• 5. Sometimes act as a Keyworker for people receiving interventions.
• 6. Plan, deliver and evaluate defined, non-profession specific specialist therapeutic interventions.
• 7. Plan, deliver and evaluate Nursing Interventions as a member of a MDT.
• 8. Work as a MDT member, supporting it to function effectively; provide leadership and collaborate with other professionals.
• 9. Deliver a range of activities/interventions to support Carers.
• 10. Maintain own workload and plan others’ workload daily, using diaries/scheduling and electronic resources.
• 11. Protect individuals from abuse and harm per local safeguarding policies, share information appropriately.
• 12. Coordinate/provide protective interventions as outlined in the protection plan.
• 13. Undertake active clinical leadership and supervision within the team.
• 14. Maintain the single health and social care record in paper and electronic form.
• 15. Report/record all activity within agreed timeframes related to information, reporting and performance.
• 16. Participate in management, caseload and clinical supervision as per trust policy.
• 17. Provide mentoring/training for others, ensuring safe learning environments for student assessment and objectives.
