Job summary

Pennine Care NHS FT in Rochdale is seeking a Senior Mental Health Practitioner to join the Heywood, Middleton and Rochdale Neighbourhood Mental Health Team. You will work in the community to help individuals access mental health services, navigate resources, and improve wellbeing. The role requires RMN/RNMH/Social Worker qualification, post-registration experience, and ability to deliver brief interventions and risk management.

Qualifications

• Comprehensive Risk Assessment and Formulation Skills.
• Ability to construct effective care and risk management plans.
• Ability to deliver a range of evidence based interventions.
• Comprehensive IT Skills.
• Ability to form productive working relationships with service users, carers and multi-disciplinary staff.

Responsibilities

• Offer initial conversation and biopsychosocial assessments.
• Collaboratively engage in person-centred next steps plans.
• Screen referrals and devise risk management plans where required.
• Provide brief interventions (max 12 weeks) including coping strategies and motivation techniques.
• Support medication processes and promote interagency collaboration.
• Review treatment plans for complex cases not suited to primary care.
