Job summary

Pennine Care NHS Foundation Trust is seeking a Mental Health Practitioner for the Community Mental Health Team in Rochdale. The role sits in the Neighbourhood Mental Health Team (NMHT) and delivers triage, assessments and short-term interventions for adults and older adults with complex mental health needs.

You will work in a multidisciplinary setting, collaborating with GPs, PCNs and social care to ensure timely access to the right care.

Qualifications

• NMC registration or equivalent is required.

Responsibilities

• Triage referrals for adults and older adults and facilitate onward transfer.
• Carry out a clinical bio-psychosocial assessment including risk assessment.
• Develop personalised care plans with patients and carers.
• To provide short-term psychotherapeutic interventions and liaise with keyworkers.
