Job summary

The Pennine Care NHS Foundation Trust is seeking a Clinical Practitioner to join the Neighbourhood Mental Health Team in Rochdale. You will provide short-term, evidence-based support, triage referrals, and carry out comprehensive assessments as part of a multidisciplinary team.

You will work with local partners to ensure timely access to the right care, develop personalised care plans, and promote recovery and self-management in adults and older adults with complex mental health needs.

Qualifications

• Registration with NMC or equivalent.
• Experience in mental health care is desirable.
• Ability to work with adults and older adults.

Responsibilities

• Triage referrals for adults and older adults.
• Carry out clinical bio-psychosocial assessments and risk assessments.
• Identify when cases need specialist transfer.
• Develop personalised care plans with patients and carers.
• Provide short-term psychotherapeutic interventions and link with keyworkers.
