• Completion of Preparation for Mentorship / Practice Teacher/, is desirable together with significant post-registration experience.

Responsibilities

• Conduct biopsychosocial assessments and risk assessments as required.
• Collaborate with service users to develop person-centred next steps and care plans.
• Provide brief interventions (up to 12 weeks) and promote self-management with signposting to community services.
