Job summary

Pennine Care in Stockport is inviting applications for a Community Mental Health Practitioner. You will provide holistic assessment, interventions, risk assessments and care plans for adults with severe and enduring mental health conditions within the CMHT.

The post offers opportunities to supervise staff and students, work as part of an MDT, and contribute to recovery and social inclusion across Greater Manchester communities.

Qualifications

• Professional qualification Social Work / Mental Health Nursing / Occupational Therapy.
• Registration with HCPC / NMC.
• Experience in community mental health work.
• Willingness to undertake further training relevant to the practitioner role.
• Evidence of post-basic qualification and training.
• PSI Training.
• MSLAP or equivalent Mentor training.

Responsibilities

• Act as the Community Mental Health Practitioner and key worker for an identified group of clients, in a recovery and social inclusion focused service.
• Provide supervision to staff and students placed with the team.
• Carry out holistic assessments, interventions, risk assessments and care planning for caseload.
• Administer depot medication for clients unable to attend depot clinic.
• Follow the job description and personal specification for main duties.
