Responsibilities

• Monitor and coordinate calls escalated from First Responders and Assistant Practitioners.
• Develop care plans for regular callers in collaboration with the team.
• Lead frequent attender management groups and review care plans.
• Collate data on frequent attenders and present trends.
• Offer immediate support and signpost to appropriate services.
• Collaborate with agencies such as CAS, GM Police, NWAS, and Local authority.
