Job summary

Cheshire and Wirral Partnership NHS Foundation Trust in Wirral is seeking an experienced Learning Disability nurse to join the CHC/CC/Complex Care team. The role requires working knowledge of the Mental Capacity Act, Safeguarding, Deprivation of Liberty Safeguards, and experience of assessments, case and risk management, with a personalised care approach.

Qualifications

• Degree or equivalent level of experience in health or social care.
• NMC Registration - Learning Disabilities background.
• Evidence of post registration study in specialist area.
• Desirable: Leadership qualification/experience.

Responsibilities

• Meet ongoing health care needs for LD clients with CHC assessments and funding coordination.
• Provide LD nurse case management for commissioning and bespoke care packages.
• Coordinate health funding decisions with CHC or joint funding arrangements.
• Develop and monitor effective assessment, case management and review processes.
• Advise CHC/CQ teams on service users with learning disabilities.
• Collaborate with NHS Trusts, GP practices, Local Authorities and providers.
