Job summary

The Cheshire and Wirral Partnership NHS Foundation Trust is seeking a Specialist Nurse in the complex care team to support continuing healthcare for people with learning disabilities. The role involves health needs assessments, coordinating multi-disciplinary reviews and case managing a complex caseload to secure NHS or joint funding.

You will require NMC registration with a learning disabilities background, and experience of coordinating CHC assessments and safeguarding in emotionally demanding

Qualifications

• Experience in assessing healthcare needs for learning disabilities.
• Ability to coordinate multidisciplinary reviews and funding assessments.
• Knowledge of CHC funding processes and Personal Health Budgets.

Responsibilities

• Coordinate NHS funding assessments for CHC.
• Manage complex caseloads across NHS, council, and providers.
• Lead and support CHC teams and reviews in complex cases.
• Develop and monitor assessment and review systems for learning disability services.
• Provide specialist case management and commissioning for complex health needs.
