Qualifications

• Registered Learning Disability nurse with experience in multi-agency work.
• Working knowledge of Mental Capacity Act, Safeguarding and Deprivation of Liberty Safeguards.
• Experience of health needs assessments and case/risk management.

Responsibilities

• Undertake health needs assessments under the NHS Continuing Healthcare (CHC) framework.
• Facilitate multi-disciplinary assessments and reviews within a team.
• Support individuals with case management and staff leadership.
• Coordinate NHS funding assessments for CHC and NHS Funded Nursing Care (FNC).
