Job summary

Central London Community Healthcare NHS Trust is hiring a Learning Disabilities Clinical Assessor to join the CHC team in North West London. The post is suited to a Registered Learning Disabilities Nurse (RNLD) or experienced Allied Health Professional with a passion for improving outcomes for people with learning disabilities and complex health needs.

You will assess, monitor and review individuals under the National Framework for NHS Continuing Healthcare, working with service users, families

Qualifications

• Relevant nursing or allied health qualification and registration.
• Experience in complex multidisciplinary settings is desirable.
• Knowledge of NHS Continuing Healthcare procedures and legislation.

Responsibilities

• Undertake NHS Continuing Healthcare assessments, reviews and monitoring visits.
• Apply the National Framework for NHS Continuing Healthcare in decision making.
• Collaborate with multidisciplinary teams, providers and local authorities.
• Gather and analyse information to inform assessments and reviews.
• Support person-centred care planning and responsiveness to needs.
• Provide guidance on NHS Continuing Healthcare processes and eligibility.
• Contribute to safeguarding and risk management activities.
• Maintain accurate clinical records and documentation.
• Participate in service improvement and delivery of high quality CHC services.
• Balance workload, work independently and with the team.
