Job summary

Cheshire and Wirral Partnership, NHS Foundation Trust, is seeking an experienced Learning Disability (LD) nurse to work within the CHC/CC/Complex Care team. The role requires knowledge of MCA, Safeguarding and DoLS, and experience with assessments, case and risk management, and personalised care planning.

The post holder will coordinate health needs assessments and funding processes, lead LD nurse case management for complex health needs, and liaise with CHC teams and Local Authorities to secure

Qualifications

• Registered Learning Disability nurse with knowledge of MCA, Safeguarding and DoLS.
• Experience undertaking health needs assessments and multi-disciplinary reviews.
• Proven case management and risk assessment skills in LD settings.
• Ability to work autonomously and in multi-agency environment.
• Knowledge of NHS CHC/FNC and Personal Health Budgets.

Responsibilities

• Undertake health needs assessments and facilitate multidisciplinary reviews for CHC funding.
• Provide LD nurse case management for complex health needs across Wirral.
• Coordinate funding processes with CHC or Local Authority joint funding.
• Develop and monitor effective inter-organisational working relationships.
• Support CHC/CQ teams and review systems for LD service users.
