Royal United Hospitals Bath NHS Foundation Trust is seeking a senior nurse to lead RUH@Home, delivering person-centred care in patients' homes with complex needs. You will assess, plan and evaluate care, exercise clinical leadership, and coordinate caseloads to maintain patient flow and safe discharge.
You will work with a multidisciplinary team, ensuring safety, documentation and compassionate care while supporting ongoing improvements in the virtual ward model.
Deliver hospital-level care where it can make a real difference: in a patient's own home. RUH Hospital at Home (RUH@Home) is our consultant-led virtual ward, supporting patients who would otherwise need to be admitted to hospital, or who can safely return home earlier with the right clinical support around them. The service works closely with primary, community and social care partners to ensure safe transitions of care. You will join a multidisciplinary team of doctors, nurses, therapists, therapy assistants and healthcare assistants providing personalised acute care in patients' homes, seven days a week between 08:00 and 18:00. In this key role you will be responsible for the assessment, planning, delivery and evaluation of care for patients with acute and complex needs within their own homes. Working autonomously, you will provide expert clinical decision-making, manage clinical risk and support the recognition and escalation of patient deterioration. As a senior member of our multidisciplinary team, you will coordinate the day-to-day delivery of the nursing service, support patient flow and provide clinical leadership to other staff. You will contribute to service development, quality improvement and the delivery of safe, effective and compassionate care within the virtual ward model., Patient Care Responsibilities
