Job summary

Royal United Hospitals Bath NHS Foundation Trust is recruiting for a Nurse within RUH@Home, delivering consultant-led care in patients' homes across Bath and surrounding areas. You will assess, plan and deliver personalised nursing care with autonomy, supporting early discharge and avoiding unnecessary hospital stays.

Qualifications

• Assess, plan, deliver and evaluate individualised nursing care for patients in home settings.
• Work autonomously with clinical judgement and escalate when required.
• Deliver medicines and treatments safely and accurately.
• Collaborate with RUH@Home team and external partners to ensure coordinated care.
• Support admission avoidance and early discharge where appropriate.

Responsibilities

• Provide home-based nursing assessments and care plans.
• Administer medications and monitor patient status.
• Maintain accurate clinical records and contribute to safety and quality improvements.
• Support student supervision and ongoing professional development.
