Job summary

Northern Care Alliance NHS Foundation Trust is recruiting a Band 7 Specialist Frailty Therapist based at Oldham Royal Hospital. The role supports frailty patients presenting to ED with a focus on rapid CGA, early discharge planning and 7-day service delivery.

You will work within a multidisciplinary team to ensure coordinated assessment, timely discharges and frailty education across hospital and community settings.

Qualifications

• Experience in frailty pathways and CGA assessment.
• Ability to work within a multidisciplinary team in ED/ Frailty services.
• Strong clinical and organizational skills for complex discharges.

Responsibilities

• Provide clinical support to frail older patients and families across the pathway.
• Balance clinical and operational duties within a seven-day roster.
• Deliver early CGA and coordination with community services for discharges.
• Lead timely assessments and contribute to frailty service improvements.
